Use this form to request a new domain name service (DNS) entry to be associated with a server or to modify an existing domain name entry.
ITS reserves the right to deny any DNS entry request based upon availability and appropriateness of the request.
Most requests should be for DNS entries within the "niu.edu" domain. This form can also be used to obtain custom domain names, e.g., ".org", ".com", etc. which are available through ITS for a charge. The approval of your requested non-"niu.edu" domain name is determined by the appropriate top level domain registry operators, not NIU or ITS.

This is the existing path for the website that will be the target for new domain name. For example, if the current uri to the site is "http://www.niu.edu/mascot/", then enter "mascot/" (without the quotes).
Default files (such as "index.shtml", "default.asp", etc.) should be used in these folders, so it is not necessary to specify "mascot/index.shtml". DNS names can only be assigned to websites with default files. (Thus, a target of "mascot/huskie.shtml" could not be assigned a DNS name.)
This path must already exist in production in order for the request to be processed. Please do not submit this form until websites in WebCRMA exist in production.
